I've been sick, but no money to go to the doctor. My employer said I need a doctors note, and if I can't provide it. It's job abandonment?

You must speak/visit the employer to make them aware of the conditions and ordeal.

As long as you keep them informed(written),it surely is not an abandonment and can be contested in case if your employer thinks otherwise.
